Als u uw medicatie niet in de lijst ziet, betekent dit niet dat er geen geneesmiddelenprogramma beschikbaar is. Het is belangrijk dat u de website van de geneesmiddelenfabrikant bezoekt om erachter te komen.
U kunt het aanvraagformulier meestal rechtstreeks op de webpagina van het farmaceutische bedrijf vinden. Gewoon afdrukken.
Opmerking: sommige van deze bedrijven spreken misschien liever rechtstreeks met uw arts.
